Patient Portal Login

Dr. Condrell uses an electronic health record system called e-MDs. As a Privia member, you have access to the e-MDs "Patient Portal" where you can:

  • Send a secure message to Dr. Condrell
  • Send a secure message to a Privia Health Advisor
  • View your health records
  • View your latest test results
  • Request an appointment
  • Request a prescription refill

PLEASE NOTE that for security reasons, the username and password you use to access the e-MDs Patient Portal is different than the username and password used to log into your Privia membership account and Mayo Clinic EmbodyHealth site. If you have forgotten your username and password for the e-MDs Patient Portal, you can call a Privia Advisor at 888-774-8428 during normal business hours and we can reset it for you (or leave us a voicemail outside of business hours and we'll get back with you as quickly as possible).
